Over three episodes, basic concepts, history, and the Ranbaxy case gradually brought us into the complex institutions and industry of generic drugs. Here we return to China and the issues closest to us.

Generics hold the largest share of China’s drug market, but domestic generics underwent a long rebuilding of standards: from legacy problems in early approvals to consistency evaluation launched in 2016 and centralized procurement in 2018. The goal was to meet appropriate quality and efficacy standards while benefiting more people through lower prices.

Procurement offered new answers to hospitals’ dependence on drug markups and the difficulty and expense of treatment, while exchanging volume for price drove costs down. But after no originator drug won a place in the tenth round for the first time,the quality of procured generics and the choice available to doctors and patients returned to public debate.Meanwhile,questions about hospitals’ limited insurance funds, domestic manufacturers facing low-price competition, implementation of the MAH system, and originator drugs possibly leaving some markets made matters more complex.
